Use the Preliminary Sign-up Sheet to survey potential interest

Using the attached sign-up sheet, gather at least 25 -40 names of individuals who would be willing to donate blood.

The sign-up sheet is a survey only, not a committment to host a blood drive. The preliminary sign-up sheet will help you decide how many potential donors you have and a possible date and time.

Once you know the interest level in your organization, it's time to contact your BloodSource Account Manager at (800) 995-4420.

Remember, when you host a blood drive, your commitment is only to find the donors. We'll do the rest!
